# Amorolfine Nail Lacquer
## Overview
Amorolfine is a morpholine-derivative topical antifungal agent that acts by inhibiting fungal cell wall synthesis through the interruption of ergosterol biosynthesis. It is formulated as a nail lacquer (typically 5%).
## Primary Indications
Treatment of mild distal and lateral subungual onychomycosis caused by dermatophytes, yeasts, and molds.
## Adult Dosing
Apply to the affected finger or toenail once or twice weekly. Treatment should continue until the nail has regenerated and the infected area is completely healed (typically 6 months for fingernails, 9–12 months for toenails).
## Pediatric Dosing
Safety and efficacy in children have not been well-established. Use in the pediatric population is generally "off-label" and should be managed under specialist supervision.
## Dose Adjustments
* **Renal/Hepatic Impairment:** No dose adjustments are required due to minimal systemic absorption.
* **Elderly:** No dose adjustments required.
## Contraindications
Hypersensitivity to amorolfine or any component of the lacquer.
## Adverse Effects
* **Local:** Burning sensation, erythema, pruritus, or skin irritation around the nail plate.
* **Nail changes:** Nail discoloration, brittleness, or ingrown toenails (rare).
## Key Drug Interactions
No clinically significant systemic interactions reported due to negligible systemic absorption. Avoid applying other nail cosmetic products (e.g., nail polish) to the treated nail during therapy.
## Monitoring
* Assess nail growth and reduction in fungal involvement every 3 months.
* Monitor for signs of local irritation or hypersensitivity.
* If no improvement is noted after 6 months, verify diagnosis and consider systemic therapy.
## Clinical Pearls
* **Application:** Prior to the first application, affected parts of the nail (especially the surface) should be filed down as thoroughly as possible using the provided nail file. The surface must then be cleaned and degreased with an alcohol swab. Repeat filing with every subsequent application.
* **Hygiene:** Files used for infected nails must not be used on healthy nails.
* **Penetration:** Avoid contact with skin surrounding the nail, as the lacquer can cause irritation. Use of protective gloves is recommended when working with organic solvents (e.g., paint thinners, detergents).
