# Amorolfine Nail Lacquer
## Overview
Amorolfine is a morpholine-derivative topical antifungal agent. It disrupts fungal cell membrane synthesis by inhibiting delta-14-reductase and delta-7-to-delta-8-isomerase, leading to the depletion of ergosterol and accumulation of atypical sterols.
## Primary Indications
Treatment of onychomycosis (distal and lateral subungual) caused by dermatophytes, yeasts, and molds.
## Adult Dosing
Apply to affected fingernails or toenails **once weekly**.
* **Duration:** Continue until the nail is regenerated and the affected area is completely healed.
* **Typical Course:** Fingernails: ~6 months; Toenails: 9–12 months.
* **Clinical Review:** Assess progress every 3 months.
## Pediatric Dosing
Safety and efficacy have not been established in children. Use is generally avoided in pediatric populations unless directed by a specialist.
## Dose Adjustments
* **Renal/Hepatic Impairment:** No dose adjustments required due to negligible systemic absorption.
* **Elderly:** No dose adjustment necessary.
## Contraindications
Hypersensitivity to amorolfine or any components of the lacquer.
## Adverse Effects
* **Common:** Burning at the site of application, erythema, pruritus, or skin irritation around the nail bed.
* **Rare:** Contact dermatitis, nail disorders (discoloration, splitting, brittleness).
## Key Drug Interactions
No clinically significant systemic drug interactions have been reported due to minimal systemic absorption when applied topically as directed. Avoid the concurrent use of other topical nail products (e.g., cosmetic nail polish or artificial nails) during treatment.
## Monitoring
* Monitor for signs of local irritation or allergic contact dermatitis.
* Evaluate nail growth and clearance every 3 months. Discontinue if no improvement is noted after 6 months.
## Clinical Pearls
* **Application Technique:** Before the first application, file the affected nail surface as thoroughly as possible. Clean and degrease the nail surface using an alcohol swab before each subsequent application to remove any residues.
* **Product Maintenance:** Ensure the bottle is tightly closed after use. If using cosmetic nail varnish, wait at least 24 hours after amorolfine application before applying paint; remove polish carefully before the next amorolfine application.
* **Special Considerations:** Use a fresh spatula for each nail to prevent cross-contamination. Avoid contact with solvents (e.g., white spirit, paint thinner) — wear impermeable gloves during household cleaning.
