# Ambroxol
## Overview
- **Classification**: Mucolytic, secretolytic, expectorant
- **Mechanism**: Increases bronchial secretions, reduces mucus viscosity, and stimulates pulmonary surfactant production, facilitating mucus clearance.
## Primary Indications
1. **Acute Bronchopulmonary Diseases**: Relief of symptoms associated with abnormal mucus secretion and impaired mucus transport.
2. **Chronic Bronchopulmonary Diseases**: As an adjunct for conditions like COPD and chronic bronchitis.
3. **Sore Throat**: Local anesthetic effect for symptom relief (lozenges).
## Adult Dosing
### Standard Dosing
**Acute & Chronic Bronchopulmonary Diseases**
- **Dose**: **30 mg** (tablet) or **10 mL** (syrup)
- **Frequency**: **3 times daily** for 2-3 days, then **2 times daily**
- **Route**: Oral
- **Maximum**: **90 mg/day**
**Sore Throat (Lozenges)**
- **Dose**: **20 mg** (lozenge)
- **Frequency**: Up to **6 times daily**
- **Route**: Oral (buccal absorption)
- **Duration**: Do not use for more than **3 days** without medical advice.
### Dose Adjustments
- **Renal Impairment**: Reduce dose or extend intervals in severe renal impairment (e.g., CrCl < **30 mL/min**).
- **Hepatic Impairment**: Reduce dose or extend intervals in severe hepatic impairment.
- **Elderly Patients**: No specific adjustment, but monitor for reduced renal/hepatic function.
## Pediatric Dosing
### Neonates (0-28 days)
- **Dose**: Not routinely recommended. If essential, **7.5 mg** (oral solution)
- **Frequency**: **2-3 times daily** (approx. **1.2-1.6 mg/kg/day** divided)
- **Maximum**: **22.5 mg/day**

### Infants (1-12 months)

- **6-12 years**: **30 mg** (oral solution/tablet) **2 times daily** or **15 mg** **3 times daily**.
- **Maximum**: **60 mg/day** for 6-12 years (or **90 mg/day** as adult lower limit).
### Adolescents (13-18 years)
- **Dose**: Follow adult dosing recommendations.
- **Maximum**: **90 mg/day**
## Safety Information
### Contraindications
- **Absolute**: Hypersensitivity to ambroxol or related compounds (bromhexine).
- **Absolute**: History of severe cutaneous reactions (e.g., Stevens-Johnson Syndrome, Toxic Epidermal Necrolysis).
- **Relative**: Peptic ulcer disease (use with caution).
### Common Adverse Effects
- **Common (1-10%)**: Nausea, taste disturbance (dysgeusia), oral/pharyngeal hypoesthesia.
- **Uncommon (0.1-1%)**: Vomiting, diarrhea, dyspepsia, abdominal pain, dry mouth.
- **Serious but Rare**: Anaphylactic reactions, angioedema, severe skin reactions (SJS, TEN).
### Key Drug Interactions
- **Cough Suppressants**: Not recommended; may lead to dangerous accumulation of bronchial secretions.
- **Antibiotics (e.g., Amoxicillin, Cefuroxime, Erythromycin, Doxycycline)**: Ambroxol can enhance penetration of these antibiotics into bronchial secretions. (Generally considered beneficial).
## Monitoring & Follow-up
- **Before Treatment**: No specific baseline tests usually required.
- **During Treatment**: Monitor for signs of gastrointestinal upset or skin reactions.
- **Clinical Signs**: Watch for worsening cough, dyspnea, or any new skin rash (discontinue immediately).
## Clinical Pearls
- 💡 **Hydration**: Ensure adequate fluid intake to maximize mucolytic effect.
- 💡 **Timing**: Can be taken with or without food.
- 💡 **Sputum**: Advise patients that increased sputum production is expected.
