# Ambrodil
## Overview
- **Classification**: Mucolytic, Expectorant
- **Mechanism**: Ambroxol works by stimulating serous gland cells, increasing bronchial secretions, and reducing mucus viscosity. It enhances mucociliary clearance, making it easier to expel phlegm.
## Primary Indications
1. **Acute & Chronic Bronchopulmonary Diseases**: Conditions associated with abnormal mucus secretion and impaired mucus transport.
2. **Bronchitis**: Acute and chronic forms with productive cough.
3. **Asthma Exacerbations**: To aid in mucus clearance.
## Adult Dosing
### Standard Dosing
**Acute & Chronic Bronchopulmonary Diseases**
- **Dose**: **30 mg**
- **Frequency**: Three times daily
- **Route**: Oral (Tablet or Syrup)
- **Duration**: As per clinical need, typically 7-14 days for acute conditions.
**Extended-release formulation**
- **Dose**: **75 mg** (Extended-release capsule)
- **Frequency**: Once daily
- **Route**: Oral
### Dose Adjustments
- **Renal Impairment**: No specific dose adjustment generally required, but caution with severe impairment due to potential accumulation.
- **Hepatic Impairment**: No specific dose adjustment generally required, but caution with severe impairment.
- **Elderly Patients**: No specific dose adjustment needed. Use standard adult dosing.
## Pediatric Dosing
### Neonates (0-28 days)
- **Note**: Ambroxol is generally **not recommended** for neonates due to limited safety data. Consult a pediatrician.
### Infants (1-12 months)
- **Dose**: **2.5 mL** syrup (**7.5 mg**)
- **Frequency**: Twice daily
- **Maximum**: **15 mg/day**
- **Special Notes**: Use 15 mg/5 mL syrup concentration for accurate dosing.
### Children (1-5 years)
- **Dose**: **2.5 mL** syrup (**7.5 mg**)
- **Frequency**: Three times daily
- **Maximum**: **22.5 mg/day**
### Children (6-12 years)
- **Dose**: **5 mL** syrup (**15 mg**)
- **Frequency**: Two to three times daily
- **Maximum**: **45 mg/day**
### Adolescents (13-18 years)
- **Dose**: **30 mg**
- **Frequency**: Two to three times daily
- **Maximum**: **90 mg/day** (Adult dose)
## Safety Information
### Contraindications
- **Absolute**: Hypersensitivity to Ambroxol or other components.
- **Absolute**: Rare hereditary conditions incompatible with excipients (e.g., fructose intolerance for some syrups).
- **Relative**: History of severe allergic reactions to mucolytics.
### Common Adverse Effects
- **Common (1-10%)**: Nausea, vomiting, diarrhea
- **Common (1-10%)**: Dyspepsia, heartburn
- **Common (1-10%)**: Headache, dizziness
- **Serious but Rare**: Severe skin reactions (e.g., Stevens-Johnson Syndrome, Toxic Epidermal Necrolysis). Discontinue immediately if rash occurs.
- **Serious but Rare**: Anaphylactic reactions.
### Key Drug Interactions
- **Antibiotics**: Concomitant use with antibiotics (e.g., amoxicillin, cefuroxime, erythromycin) may lead to increased antibiotic concentration in bronchial secretions.
- **Antitussives**: Avoid concurrent use with cough suppressants as it may lead to a dangerous build-up of secretions.
## Monitoring & Follow-up
- **Before Treatment**: Assess cough type (productive vs. dry).
- **During Treatment**: Monitor for clinical improvement in mucus clearance and cough. Watch for adverse effects, especially GI upset or skin reactions.
- **Clinical Signs**: Look for reduction in sputum viscosity and ease of expectoration.
## Clinical Pearls
- 💡 **Tip 1**: Ensure adequate hydration when taking Ambrodil to help further thin mucus.
- 💡 **Tip 2**: Ambroxol is most effective for productive coughs. It is not indicated for dry cough.
- 💡 **Tip 3**: Instruct patients to discontinue use and seek immediate medical attention if a skin rash (with or without blistering) or mouth sores develop.
