# Allegra (fexofenadine)
## Overview
Fexofenadine is a second-generation antihistamine used for the symptomatic relief of allergic conditions. It is a non-sedating H1-receptor antagonist.
## Primary Indications
* Seasonal allergic rhinitis (hay fever)
* Chronic idiopathic urticaria (hives)
## Adult Dosing
* **Seasonal allergic rhinitis:** 60 mg twice daily or 180 mg once daily.
* **Chronic idiopathic urticaria:** 60 mg twice daily.
## Pediatric Dosing
* **Children 6 to 11 years:**
* Seasonal allergic rhinitis: 30 mg twice daily.
* Chronic idiopathic urticaria: 30 mg twice daily.
* **Children 6 months to <6 years:**
* Seasonal allergic rhinitis: 15 mg twice daily.
* Chronic idiopathic urticaria: 15 mg twice daily.
* **Infants <6 months:** Safety and efficacy not established.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment.
## Contraindications
Hypersensitivity to fexofenadine or any component of the formulation.
## Adverse Effects
Common adverse effects include headache, dizziness, nausea, and fatigue. Generally well-tolerated and less sedating than first-generation antihistamines.
## Key Drug Interactions
* **Antacids containing magnesium or aluminum:** May decrease the absorption of fexofenadine. Separate administration by at least 2 hours.
* **Erythromycin and ketoconazole:** May increase fexofenadine plasma concentrations. The clinical significance is unclear, but caution is advised, especially in pediatric patients.
## Monitoring
Monitor for symptom relief and the absence of significant adverse effects.
## Clinical Pearls
* Fexofenadine does not typically cause significant drowsiness, making it a good choice for patients who need to remain alert.
* Absorption can be affected by food; however, consistent administration with or without food is acceptable.
