# Fexofenadine
## Overview
Fexofenadine is a second-generation H1 antihistamine used for the symptomatic relief of allergic conditions. It is a non-sedating alternative to first-generation antihistamines.
## Primary Indications
* Seasonal allergic rhinitis (hay fever)
* Chronic idiopathic urticaria (hives)
## Adult Dosing
* **Seasonal Allergic Rhinitis:** 180 mg once daily.
* **Chronic Idiopathic Urticaria:** 180 mg once daily.
## Pediatric Dosing
* **Seasonal Allergic Rhinitis & Chronic Idiopathic Urticaria:**
* **6 to 11 years:** 30 mg twice daily.
* **12 years and older:** 180 mg once daily.
* **Note:** Oral suspension is available for younger children. Dosing for children under 6 months is not established.
## Dose Adjustments
No dose adjustment is typically required for renal or hepatic impairment.
## Contraindications
Hypersensitivity to fexofenadine or any component of the formulation.
## Adverse Effects
Generally well-tolerated. Common adverse effects include:
* Headache
* Dizziness
* Nausea
* Fatigue
## Key Drug Interactions
* **Antacids containing magnesium or aluminum:** May decrease fexofenadine absorption. Separate administration by at least 2 hours.
* **Fruit juices (e.g., orange, grapefruit, apple):** May decrease fexofenadine absorption. Advise patients to take with water.
## Monitoring
Monitor for symptom relief and potential adverse effects.
## Clinical Pearls
* Fexofenadine is the active metabolite of terfenadine.
* Non-sedating, making it suitable for patients who need to remain alert (e.g., operating machinery, driving).
* Take consistently, either with or without food.
