# Fexofenadine
## Overview
Fexofenadine is a second-generation, non-sedating antihistamine used for the symptomatic relief of allergic conditions. It is a selective peripheral H1 receptor antagonist.
## Primary Indications
* Seasonal allergic rhinitis (hay fever)
* Chronic idiopathic urticaria (hives)
## Adult Dosing
* **Seasonal allergic rhinitis:** 60 mg orally twice daily or 180 mg orally once daily.
* **Chronic idiopathic urticaria:** 60 mg orally twice daily.
## Pediatric Dosing
* **6 months to 11 years - Seasonal allergic rhinitis and Chronic idiopathic urticaria:**
* 30 mg orally twice daily.
* **12 years and older:** Same as adult dosing.
## Dose Adjustments
No dose adjustment is typically needed in patients with renal or hepatic impairment.
## Contraindications
Hypersensitivity to fexofenadine or any component of the formulation.
## Adverse Effects
Generally well-tolerated. Most common adverse effects include headache, dizziness, nausea, and fatigue.
## Key Drug Interactions
* **Antacids containing magnesium or aluminum:** May decrease fexofenadine absorption. Administer fexofenadine at least 15 minutes before or 2 hours after antacids.
* **Erythromycin and ketoconazole:** May increase fexofenadine plasma concentrations, but clinically significant adverse effects are unlikely.
## Monitoring
Monitor for symptom relief. Assess for potential adverse effects, though significant monitoring is typically not required.
## Clinical Pearls
* Fexofenadine is a metabolite of terfenadine and does not possess the cardiac side effects associated with terfenadine.
* Oral suspension for pediatric use requires careful measurement to ensure accurate dosing.
