# Lennox-Gastaut Syndrome (LGS)
## Overview
Lennox-Gastaut syndrome is a severe, childhood-onset epilepsy syndrome characterized by multiple seizure types (tonic, atonic, absence), slow spike-wave EEG patterns, and intellectual impairment. Treatment is often refractory, requiring polypharmacy.
## Primary Indications
Adjunctive treatment of seizures associated with LGS in patients typically ≥1 to ≥2 years of age.
## Adult Dosing
Relevant medications for LGS generally follow titration schedules to minimize CNS side effects:
* **Rufinamide:** Start 400–800 mg/day in two divided doses; increase by 400–800 mg/day every 2 days to max 3200 mg/day (or 45 mg/kg/day).
* **Clobazam:** Start 5 mg/day; increase to 20 mg/day over 2 weeks. Max: 40 mg/day.
* **Cannabidiol (Epidiolex):** Start 2.5 mg/kg twice daily (5 mg/kg/day); increase to 5 mg/kg twice daily (10 mg/kg/day) after 1 week. May increase to 20 mg/kg/day based on tolerance.
* **Topiramate:** Start 25 mg/day; titrate weekly by 25–50 mg. Target 200–400 mg/day.
* **Felbamate:** Start 1200 mg/day in three divided doses; increase in 1200 mg increments every week. Max: 3600 mg/day (requires strict informed consent).
## Pediatric Dosing
Dosing is weight-based (usually ≥1 year):
* **Rufinamide:** 10 mg/kg/day initially, divided BID; increase by 10 mg/kg/day every 2 days. Max: 45 mg/kg/day (not to exceed 3200 mg/day).
* **Cannabidiol:** Same as adult (mg/kg basis).
* **Clobazam:** Weight <30 kg: Start 5 mg/day; target 0.2–0.3 mg/kg/day. Weight ≥30 kg: Start 10 mg/day; target 20 mg/day.
* **Rufinamide (1-4 years):** Often requires specialized titration due to higher clearance rates in younger children.
## Dose Adjustments
* **Renal:** Clobazam and Topiramate require dose reduction in severe impairment. Rufinamide is not recommended in severe renal impairment.
* **Hepatic:** Adjustments mandatory for Cannabidiol (lower starting/maintenance doses) and Clobazam. Avoid Felbamate or use with extreme caution due to hepatotoxicity.
## Contraindications
* **Rufinamide:** Familial Short QT syndrome.
* **Felbamate:** History of hepatic impairment or bone marrow suppression.
* **General:** Known hypersensitivity to the specific agent or its excipients.
## Adverse Effects
* **Common:** Somnolence, fatigue, dizziness, ataxia, irritability, and decreased appetite.
* **Specific:**
* **Rufinamide:** Shortened QT interval.
* **Cannabidiol:** Elevated transaminases (ALT/AST), diarrhea.
* **Felbamate:** Aplastic anemia, acute liver failure (Black Box Warnings).
* **Topiramate:** Metabolic acidosis, nephrolithiasis, cognitive slowing ("dopamax").
## Key Drug Interactions
* **Clobazam:** Potent CYP2C19 interaction (may increase concentrations of other AEDs).
* **Cannabidiol:** Significant interaction with Clobazam (increases N-desmethylclobazam levels, increasing toxicity risk) and Valproate (increases risk of ALT elevation).
* **Enzyme Inducers:** Carbamazepine, phenytoin, and phenobarbital decrease levels of many LGS-adjunct medications.
## Monitoring
* **CBC and LFTs:** Required for Felbamate and Cannabidiol.
* **ECG:** Mandatory for Rufinamide (baseline and during titration).
* **Serum Bicarbonate:** Monitor for topiramate-induced metabolic acidosis.
* **Neurologic:** Monitor for increased seizure frequency (paradoxical) and suicidal ideation (class-wide risk).
## Clinical Pearls
* **Titration:** LGS medications must be titrated slowly to avoid intolerable CNS sedation.
* **Polypharmacy:** Patients are almost always on multiple agents; focus focus on identifying the "main" driver of side effects (e.g., elevated N-desmethylclobazam).
* **Rescue:** Clobazam is frequently used as a rescue medication because of its long half-life, but tolerance can develop.
* **Local Protocol:** Dosing protocols, particularly for rapid titration or transition between agents, vary by epilepsy center; always consult local pediatric neurology guidelines.
