# Ferrous Ascorbate
## Overview
Ferrous ascorbate is an iron salt complexed with ascorbic acid. The inclusion of ascorbic acid lowers the gastric pH, which enhances the solubility, bioavailability, and absorption of elemental iron compared to other oral iron formulations.
## Primary Indications
* Treatment of iron deficiency anemia (IDA).
* Prophylaxis of iron deficiency in high-risk populations (e.g., pregnancy, chronic blood loss).
## Adult Dosing
* **Treatment of IDA:** Typical dosage is 100 mg of elemental iron equivalent once or twice daily.
* **Note:** Dosing varies significantly based on local institutional protocols and the severity of anemia (Hb level). Maintenance dosing is often lower (e.g., 50–100 mg daily).
## Pediatric Dosing
* **Prophylaxis:** 1–2 mg/kg/day of elemental iron.
* **Treatment:** 3–6 mg/kg/day of elemental iron, divided into 1–3 doses.

## Dose Adjustments

* **Levodopa/Methyldopa:** Absorption may be reduced by iron salts.
* **Calcium supplements:** Can inhibit iron absorption if taken concurrently.
## Monitoring
* **Efficacy:** Reticulocyte count (7–10 days post-initiation), Hemoglobin/Hematocrit (4 weeks post-initiation), and Ferritin (3 months post-initiation).
* **Safety:** Monitor for GI intolerance and signs of systemic iron overload in chronic therapy.
## Clinical Pearls
* **Bioavailability:** Best absorbed on an empty stomach. If GI side effects are limiting, administer with food, though this may decrease overall absorption.
* **Ascorbic Acid Synergism:** The presence of ascorbic acid maintains iron in the ferrous (Fe2+) state, facilitating better absorption in the duodenum.
* **Toxicity:** Always store out of reach of children. Even small amounts can be toxic to toddlers.
