# Dexamethasone in Chronic ITP
## Overview
Dexamethasone is a potent, long-acting glucocorticoid used for immunosuppression in immune thrombocytopenia (ITP). In chronic ITP, pulsed high-dose dexamethasone is employed to rapidly increase platelet counts and induce remission.
## Primary Indications
- Second-line therapy for adult chronic ITP (after corticosteroids or IVIG)
- First-line for newly diagnosed ITP (as an alternative to prednisone)
- Bridging therapy before splenectomy or rituximab
- Rescue therapy for bleeding or pre-surgery
## Adult Dosing

- **Aspirin/NSAIDs:** Increased gastrointestinal bleeding risk
- **CYP3A4 inducers (phenytoin, rifampin, carbamazepine):** Decrease dexamethasone efficacy
- **CYP3A4 inhibitors (ritonavir, azole antifungals):** May increase dexamethasone exposure
- **Live vaccines:** Avoid; use inactivated vaccines with caution
## Monitoring
- Baseline and periodic: platelet count, glucose, blood pressure, weight, electrolytes
- Signal for response: platelet count increase within 3-7 days of cycle
- Long-term: bone density (if repeated cycles), ophthalmologic exam, adrenal function after multiple cycles
